Hackney iCare

Targeted Preventative Services (TPS)

TPS is a new way of meeting the needs of adults in Hackney before they may require a full social care package.

It has three elements:

  • Health & Wellbeing
  • Floating Support (for residents aged 16+)
  • Hackney Volunteer & Befriending Service (HVBS).

The TPS floating support providers are One Support, Family Mosaic and SHP. Floating Support can help you for up to 12 months with issues like:

  • managing on a budget
  • finding work
  • training or education
  • getting involved in community activities
  • resolving difficult housing problems
  • support through a crisis
  • accessing other services such as social or voluntary services

The exciting health and wellbeing activities are being run by Norwood and their partners. These activities include cooking & eating sessions, work and employment skills training, yoga for mind, art therapy and walking groups.

The Volunteer & Befriending Service (HVBS), delivered by Outward, aims to tackle loneliness and social isolation in Hackney in a variety of ways. These include regular telephone contact with isolated residents, home visiting, escorting to medical appointments, and help to access local activities.

Outward is the single point of entry for all referrals to these services, as well as delivering the Volunteer & Befriending Service.
