Hackney iCare

Complete a self-referral

This service is aimed at adults who are experiencing problems looking after themselves. It is open to people 18 years or over who live in the borough of Hackney. Self-referral gives you the choice to assess your own care needs or carry out a referral on behalf of someone else. If you or someone you know might benefit from our support, please complete a self-referral to find out if we can help you.

To print off or download a self-referral form, you will need to install the latest version of Adobe Reader.

What happens next?

To receive services from us we must first assess your needs. We will offer an assessment to anyone who requests one or who appears to be in need of community care services.

The assessment is designed to identify your needs and evaluate any risks to your independence, if those needs are not met. Based on this assessment, we will decide if you have priority needs and are eligible for support from Adult Social Care.

Eligibility is determined through assessing your needs against the criteria as stated by the National Framework in the Care Act 2014.

Choose from a range of services

If you have been assessed as being eligible for support, you will be advised about a range of services matching your assessed needs. If you would prefer to arrange some services yourself, you can choose to have a cash equivalent personal budget instead.

I understand that in order to ensure the right services are provided to meet my needs, the information I provide may be shared with professionals such as district nurses, physiotherapists etc. By continuing with the self-referral, I give my consent to my information being shared for these purposes.
